TO REGISTER

1. Complete and return all forms.

2. Submit a $75.00 Non-Refundable registration fee per child.

All payments are to be made payable to: The Hide Out

3. Provide a copy of your child’s health form.

4. Obtain parent’s manual, read, and sign.


                                                                ENROLLMENT ELIGIBILITY                                                  

 Enrollment into the program will be on a first come first serve basis.

·         The children must be between the ages of 2.75 (as of September) and 5. 

·         Current enrollment will be given first opportunity to re-register for the next school year. 

·         Children must be potty trained.

REGISTRATION

     In order for registration to be considered complete, parents must:

1.  Complete registration forms.

2.  Provide Smart Start with the child's health form.

3.  Sign the following forms:

v  Contract

v  Confirmation

v  Permission to be Transported (if necessary)

v  Release of Information & Photo Release

v  Waiver Form

v  Parent manual received, read, and signed

5.  Submit all forms and a Non-refundable registration fee of $75.00 per child to:
